Joining Forms for new members to an existing scheme.

The form below is an example that has been used successfully for many years.
Because NHW schemes are not all identical, scheme Co-ordinators can modify the form to suit.

Electronic Data Authorisation Form

This form is required by the Data Protection Act. Co-ordinators and contacts fill in this form to give A&S Police permission to hold their personal details in a database. A&S Police need this information  so that they can contact NHW co-ordinators in the event of any incidents and for distribution of Ringmaster messages and alerts.
If Co-ordinators or Contact People stand down, any newly appointed people to these roles will need to complete a new Data Authorisation form.  Once completed, any newly completed forms should be returned to the PCSO for the area, who will update the information held on the A & S database.   If Co-ordinators or Contact People change their e-mail address or other contact details, completion of a new Data Authorisation form is advisable to make absolutely sure that the new details are registered.  The form should be clearly marked at the top CHANGE OF DETAILS.   Do please remember, it is important to notify your PCSO of the changes, however you choose to do it.
